    Please determine which scenario is applicable for your entity:
     

    Scenario 1
     

    If your entity is a single-member limited liability company owned by an individual, hence your Legal Representative who satisfies all standards and conditions for chief accountant also acts as the chief accountant.

    or

    If your entity is extra-small as defined in the following conditions:

    1. Agriculture, forestry and fishery/ Industry and construction sector has the total number of employees is 10 persons/less than 10 persons and total revenue of the preceding year or total capital is not higher than VND3 Billion or

    2. Trade and service sector with total number of employees not more than 10 persons and total revenue of the preceding year not higher than VND10 Billion or total capital not higher than VND3 Billion

    Please fulfil the following steps:

    1. Download and print Form A1 Form A1 – Declaration for Non-Individual Account(s) and Service(s)
    2. Complete the declaration form and to be signed by Legal Representative of company
    3. Mail original copy of Form A1 to the following address Account Services Department

    Scenario 2
     

    If your entity has already registered two groups of signatories representing for account holder and chief accountant with the Bank in compliance with prevailing regulations on opening and management of bank accounts and/ or internet banking.

    Please fulfil the following steps:

    1. Download and print Form A2 Form A2 – Declaration for Non-Individual Account(s) and Service(s)
    2. Complete the declaration form and to be signed by Legal Representative, Chief Accountant of company and any other signatories as required by your company charter/ constitution
    3. Mail original copy of Form A2 to the following address Account Services Department

    Scenario 3
     

    If you need to update your account and/ or internet banking signing/ approving mandate to meet the requirements of applicable regulations on account holder, chief accountant and authorized signatories on banking vouchers.

    Please fulfil the following steps:

    1. Download and print Form B Form B1 - Mandate for Account Operating and Banking transaction
    Form B2 - UOB Business Internet Banking Maintenance Form
    2. Complete Account/ Business Internet Banking mandate form(s). To be signed by Legal Representative, Chief Accountant of company and any other signatories as required by your company charter/ constitution
    3. Mail original copy of Form B to the following address Account Services Department
